Since 2011 the Have A Heart Clinic has provided close to a quarter of a million dollars in free healthcare services to those in need in outpatient care. That number does not include any hospital based procedures that the Have A Heart Clinic facilitates.
Healthcare Services Provided by Have A Heart Clinic
Passport Health is providing consulting support to assist Have A Heart’s efforts to plan and start a care-team approach to treating patients with the highest risk factors. With Passport’s help, the team will include a health care provider, clinical psychologist, and social worker, all committed to addressing patients’ health needs, as well as their home needs. People with 4 or more adverse childhood events are 2 times more likely to develop heart disease. We need to have ways to address that socioeconomic disparity if we are to truly make an impact in their lives. They are often hungry, untrusting and lack a way to change. With the help of Passport Health we have developed a Cardiovascular Home/care coordination approach. It involves Community Health Workers to gain trust and coordinate the care of those patients. It involves a care team center around that person and utilizing a Physician provider, nursing staff, clinical psychologist, social worker, dietician and pharmacists. A group of professionals to coordinate that patients care with the sole purpose of being patient centric to put that patient on a path to better health. Providing them with the resources and support to stay on that path.
